Abstract
Pneumatization of the maxillary sinus is variable even after birth with the alveolar recess being a foci of sinus pathologies especially of odontogenic fungal sinusitis. We present a similar case with hyper pneumatized alveolar recess that warranted a Caldwell Luc’s approach apart from FESS to clear the pathology, but after opening up the alveolar recess, an additional drilling was required at the level of floor of the nasal floor to open up the rest of the sinus. CT PNS of this patient showed a cell in the recess with bony walls opening postero superiorly unlike septation which is easily removed and which do not obstruct the view of the rest of the sinus. Thus, a new alveolar recess cell was discovered which required an additional approach to clear the pathology.
