Abstract

Introduction: Curettage adenoidectomy has some drawbacks such as leaving residuals (under curettage). Literature mentioned the commonest sites of the residual parts are the roof and posterior wall of the nasopharynx detected by using nasopharyngeal mirror or 45-degree endoscopy. This study aims to evaluate the adenoid residuals intra-operatively after routine curettage adenoidectomy regarding percentage of cases and site of residuals among sample size. Methods: A prospective observational study conducted on 250 children under 12-yearold of both sexes with nasopharyngeal adenoid at Otolaryngology department of Cairo University hospitals from Oct. 2024 to Oct. 2025. Study was approved by ethical committee of faculty of medicine, Cairo University, Egypt with IRB number (N-397- 2024). All children have been subjected to routine transoral curettage adenoidectomy using Boyle Davis mouth gag and Beckmann adenoid curette followed by intraoperative inspection of the residuals in the same set by using of the 0 and 30 and 45-degrees 4 mm rigid endoscope trans-orally. Results: In this study we reported that the commonest site of missed adenoid is roof of nasopharynx, in 61 cases (24.4%), while the least common site for incidence of missed adenoid in this study was the choana 16 cases (6.4%). Conclusion: We can conclude that routine curettage adenoidectomy carries high incidence of missed residuals, intraoperative inspection by endoscope helps visualization of missed adenoid residuals. Commonest site of missed residuals is roof of nasopharynx.
